Job Summary

Under the direction of the Program Manager, the Community Residence Counselor (CRC) provides direct patient care and participates in the therapeutic milieu within the scope of defined practice.

Essential Functions
• Provides direct patient care in accordance with the current treatment plan, under the supervision of the Nurse Manager, Clinical Coordinator, and Charge Nurse.
• Provides and supervises community activities as needed.
• Remains awake and alert at all times while on the job.
• Assists patients in maintaining and keeping various appointments.
• Completes clear, concise, and timely documentation and shift notes.
• Establishes helpful relationships with patients and maintains professional boundaries.
• Participates in patient rounds, status checks, or teams per program guidelines.
• Completes assigned patient care tasks per program guidelines and/or daily assignment sheet.
• Works effectively to manage the therapeutic milieu.
• Provides patients with direction and supervision as needed.
• Follows established hospital and program policies, procedures, and protocols.
• Communicates information regarding patient milieu/therapeutic environment to appropriate staff in a timely manner.
• Communicates observations related to medication side effects and physical and emotional changes to appropriate medical staff in a timely manner.
• Encourages patients in their activities of daily living.
• Assesses sleep patterns and assists patients to develop more appropriate sleep cycles, notifying the clinical team as necessary.
• Completes mandatory annual in-service requirements, including but not limited to CPR and de-escalation.
• Attends and participates in rounds and case conferences when appropriate.
• Maintains the physical ability to participate in patient activities.
• Supports residents with daily routines, medication reminders, meal preparation, transportation, and personal care as needed.


 

Qualifications

Education High School Diploma or Equivalent required and Bachelor's Degree Related Field of Study preferred Can this role accept experience in lieu of a degree? No Licenses and Credentials Class D Passenger Vehicle Driver's License [State License] - Generic - HR Only preferred Experience One year of patient care or related experience, preferably in a psychiatric setting 1-2 years preferred Knowledge, Skills and Abilities - Time Management. - Excellent verbal and written communication skills. - Ability to work as part of a team. - Maintains a commitment to professional growth and competency. - All employees must possess basic computer skills to use a variety of electronic or online systems (PRISM) for communication, clinical, and administrative purposes. - Familiarity with Epic is preferred. - Ability to drive, including transporting patients, may be required based on role and location.
