ABOUT THE ROLE:
Under the general supervision of the Sr. Clinical Manager, the Clinical Guide Behavioral Health RN performs clinical triage, high-risk outreach, condition support, care planning, clinical education, and care coordination to support Rightway members in navigating and accessing care.
This role supports members across both behavioral and physical health needs, including conditions such as anxiety, depression, and chronic disease. The RN serves as a trusted clinical advisor, guiding members to high-quality, appropriate care with the goal of improving clinical outcomes, enhancing the member experience, and reducing unnecessary healthcare utilization (e.g., avoidable ED visits) while promoting appropriate care settings such as primary care and behavioral health services.
WHAT YOU'LL DO:
- Perform clinical triage using evidence-based protocols and algorithms to assess member needs and direct them to the appropriate level of care safely and efficiently
- Conduct proactive, data-driven outreach to high-risk, high-cost, rising-risk members and those with identified care gaps
- Engage members in clinical and behavioral health conversations to assess needs, risks, preferences, and barriers to care
- Provide behavioral health support, including education on conditions such as anxiety, depression, and stress-related disorders
- Conduct suicide risk assessments and apply de-escalation techniques when appropriate
- Identify and address barriers preventing members from accessing preventive, primary care, and behavioral health services
- Deliver clinical navigation support, including:
- Coordinate completion of screenings and follow-up care for high-risk members
- Accurately document symptoms, assessments, clinical guidance, and member responses in a timely manner
- Act as a member advocate and trusted healthcare advisor, supporting members throughout their care journey
- Promote a high-quality member experience and retention through longitudinal engagement and education
- Collaborate cross-functionally with Navigation and internal teams to ensure seamless, coordinated care delivery
- Maintain a member-centric, customer-driven approach in all interactions
- Practice within RN scope, adhering to established care navigation protocols and clinical guidelines
WHO YOU ARE:
- RN with ADN or BSN degree
- Must have an active, unrestricted Registered Nurse (RN) multi-state license and resides in a compact state
- Holds a Psychiatric Mental Health Nursing Certification (PMH-BC)
- 5+ years of nursing experience, including:
- Experience with suicide risk assessment and de-escalation techniques
- Experience in population health, disease management, or care management preferred
- Chronic condition support experience preferred
- Strong ability to engage members in both behavioral health discussions
- Skilled in care coordination, patient education, and navigating complex healthcare systems
HOURLY RATE: $34.00-$40.86
