The Care Coordinator role is to improve health outcomes through coordinating care, patient education, enhancing communication and continuity of care, and building trust between patient and medical practitioners. The Care Coordinator will teach, counsel, and monitor patients on health issues relevant to their care. They will consult with other health care team members to coordinate the services of patient education, preventative care and disease management.
The LPN/RN will provide direct and indirect patient care in the clinic setting. He/she communicates with providers/nurse manager/co-workers as appropriate, about changes in patient’s clinical condition including results of diagnostic evaluation and changes in patient condition or symptomatology. The nurse will be able to respond quickly and accurately to changes in condition or response to treatment, and avail his or herself of all training, skills and ability commonly developed through attainment of licensure in their field. The nurse may also have inventory/ordering responsibilities in addition to clinical responsibilities, and will expected to cross train in additional areas of clinic responsibilities including laboratory rapid assays, reception and radiographic studies.
DUTIES AND RESPONSIBILITIES:
LPN:
The LPN provides direct and indirect patient care in the clinic setting. - This position is specifically assigned to work as the Chronic Care Coordinator.
- Work under direct supervision of MD, PA-C or NP to provide excellent patient care to all clients that come to Urgent Care.
- Is able to perform a head-to-toe assessment on all patients and performs reassessments as per policy. This includes: pediatric, geriatric, the general patient population, and EMTALA appropriate medical screening exams.
- Adequately assesses and reassesses pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
- Revises plan of care as indicated by the patient’s response to treatment.
- Performs waived testing (point-of-care testing) per clinical laboratory’s and the patient care unit’s policies and procedures as needed when the lab tech is unavailable and performs tests in a timely manner.
- Is able to interpret results of waived tests accurately, taking appropriate action on waived tests’ results.
- Performs patient care responsibilities considering needs specific to the standard of care for patient’s age.
- Is knowledgeable of medications and their correct administration based on age of the patient and their clinical condition.
- Follows the five medication rights and reduces the potential for medication errors.
- Formulates a teaching plan based on identified patient learning needs and evaluates effectiveness of learning, family is included in teaching as appropriate.
- Capably assists providers with procedures and performs services requiring technical and manual skills.
- Is able to perform treatments and provide services to level of licensure.
