Position Summary

CereCore is seeking to hire a U.S.-based licensed Pharmacist to join our pharmacy clinical specialist team. This role is responsible for developing medication formulary standards and building associated order sets within the electronic health record (EHR) as part of a pharmacy clinical specialist team focused on standardization and implementation. The pharmacist will apply clinical expertise and professional judgment to guide medication selection and administration methods for inclusion in facility and Enterprise formularies. The position collaborates with multidisciplinary teams to ensure comprehensive alignment across all medication management systems, including the EHR, IV infusion pump libraries, automated dispensing cabinets (ADCs), compounding verification software, and facility-specific operational needs. This is fully-remote position; however, occasional travel may be required. 

Major Responsibilities:

  • Analyze clinical, operational, and financial data to recommend safe, evidence-based, and cost-effective medication
  • inclusion/exclusion for facility and Enterprise formularies.
  • Define standard medication elements (names, strengths, routes, dose ranges, frequencies, defaults), order sets,
  • and clinical decision support (alerts, dose checks) for EHR build and maintenance.
  • Align medication records across electronic systems (ADC, IV pumps/smart pumps, compounding systems) through
  • mapping, nomenclature, concentrations, and profiles to ensure end-to-end consistency.
  • Evaluate clinical literature and guidelines to develop formulary and EHR standardization guidance; produce clear
  • summaries for physicians, nursing, pharmacy, and service line leaders.
  • Ensure compliance with company policies, regulatory standards, and medication safety practices.
  • Develop standardized documentation and tools to support medication alignment and implementation.
  • Collaborate with stakeholders and contribute to achieving goals for the medication standardization program and
  • related workstreams.
  • Work independently in a fast-paced environment to deliver results within aggressive timelines.

Education & Experience:

  • B.S. or Pharm D degree in Pharmacy Required
  • Minimum of 2 years Acute Care Pharmacy experience Required
  • Graduate of an accredited PGY-1 residency program Preferred

Licenses, Certifications, & Training:

  • Maintains current pharmacist licensure Required
