The Director of Post Acute Clinical Services is responsible for planning, directing, supervising, and monitoring the activities of both the Home Health and Hospice departments to ensure optimal patient care by achieving established goals and objectives. This role requires coordinating activities with the Medical Staff, Administration, other nursing units, families, ancillary services, and community agencies to enhance total patient care. The Director may act in the Administrator capacity in the absence of the VP/Administrator of WVUHS Home Care. The position oversees the overall organization and financial management of Home Health and Hospice operations across all service locations, including but not limited to West Virginia and Pennsylvania. The Director ensures business development enhancement and continuous improvement of Home Health and Hospice operations, efficiency, and fiscal success. The Director is responsible for ensuring Home Health and Hospice operations function according to approved policies, procedures, and standards, while maintaining a positive image within the community, as well as state and national organizations.MINIMUM QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. Current unencumbered licensure with the WV Board of Registered Professional Nurses, and other (PA and Ohio) state boards where services will be provided, as a registered professional nurse OR current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC) (within 90 days of hire).
2. Valid Driver’s License.
3. Bachelor of Science in Nursing.
EXPERIENCE:
1. Five (5) years of progressive Registered Nurse experience.
2. Two (2) years of supervisory experience.
PREFERRED QUALIFICATIONS:

1. Bachelor of Science in Nursing.
EXPERIENCE:
1. Two (2) years of previous experience in a hospital/home care/hospice leadership role (such as charge nurse, nurse manager, or documentable consistent demonstration of strong leadership skills while functioning as a staff nurse).

1. Administrative Role: Serve as Administrator.
2. Plan and Supervise: Monitor the activities of Home Health and Hospice departments to ensure optimal patient care.
3. Strategic Planning: Oversee the execution of strategic goals and plans for Home Health and Hospice services.
4. Financial Management: Supervise financial operations, ensuring efficiency, profitability, and growth.
5. Quality and Compliance: Ensure compliance with current regulatory requirements from CMS, TJC, and assure policies, procedures, and standards are current.
6. Coordinated Multidisciplinary Care: Facilitate collaboration among medical staff, administration, nursing units, families, ancillary services, and community agencies to ensure a comprehensive approach to patient care that addresses all patient needs.
7. Age-Specific Care: Ensure care is provided based on physical, psychosocial, communication, safety, and education levels for patients of all ages.
8. Team Leadership and Employee Engagement: Guide and coordinate staff activities, promoting team building, collaboration, effective problem-solving, and high levels of employee engagement to ensure a positive work environment.
9. Employee Development: Oversee employee performance evaluations, provide coaching and mentoring, and ensure competency-based training.
10. Conflict Resolution: Address conflicts and work towards resolution, ensuring a positive work environment.
11. Community Relations: Maintain a positive image within the community and professional organizations.
12. Operational Efficiency: Implement strategies to improve operational integration and efficiency across the health system. Other Duties: Perform other duties as assigned.
SKILLS AND ABILITIES:
1. Strong leadership and team development skills.
2. In-depth knowledge of post-acute care regulations (Medicare, Medicaid, CoP, JCAHO.
3. Proficiency in financial management, budgeting and operational efficiency.
