Job Title: MDS Registered Nurse (RN)
Location: Booker T. Washington Nursing Home, Louisiana
Job Summary:
The MDS RN will be responsible for conducting comprehensive assessments of residents to ensure the highest quality of care and compliance with regulatory requirements. This role involves collaboration with a multidisciplinary team to develop individualized care plans, monitor resident progress, and ensure that documentation meets the standards set forth by CMS and other governing bodies.
Key Responsibilities:
- Conduct and coordinate the Minimum Data Set (MDS) assessments for residents, ensuring accuracy and completeness.
- Develop, review, and update comprehensive care plans based on assessment findings and resident needs.
- Collaborate with nursing staff, physicians, and other healthcare professionals to implement and evaluate care plans.
- Monitor residents’ health status and respond to changes in condition, making necessary adjustments to care plans.
- Ensure compliance with federal, state, and local regulations, as well as facility policies and procedures.
- Participate in quality improvement initiatives and assist in training staff on MDS-related processes.
- Maintain accurate and confidential resident records and documentation in compliance with HIPAA regulations.
- Educate residents and their families about care plans, services, and health maintenance.
Working Conditions:

- May require occasional overtime and flexibility in scheduling.

Qualifications
Qualifications:
- Current Registered Nurse (RN) license in the state of Louisiana.
- Previous experience working with MDS in a long-term care setting preferred.
- Strong knowledge of CMS regulations and the long-term care process.
- Excellent assessment, communication, and interpersonal skills.
- Ability to work effectively as part of a team and independently.
- Proficient in electronic health record (EHR) systems and Microsoft Office Suite.
