Physical Therapist
A good physical therapist is one who can develop an achievable plan of care and help a patient through that plan. A great physical therapist is one who develops a realistic plan with measurable goals tailored to each patient’s situation. A great physical therapist works with more than just the patient, involving the physician, family and other healthcare providers as needed. A great physical therapist doesn’t just go through the motions, instead listening carefully to a patient’s concerns, monitoring progress, and adjusting plans as needed. We are looking for a great physical therapist.
WHAT YOU CAN EXPECT
Completes appropriate goal re-assessment and discharge planning.
Performs comprehensive physical therapy evaluations, treatment, and related follow-up services including discharge planning.
Develops a treatment plan with measurable goals based on patient assessment.
Collaborates with patient and other healthcare personnel or outside agencies regarding coordination of care.
Documents patient assessments and pertinent information in a timely manner.
Provides services and treatments requiring physical therapy skills according to the written plan of care and the signed plan of treatment.
Supervises Physical Therapist Assistants and Home Health Aides.
Provides physical therapy according to standards of care defined by the state practice act and other professional and governing agencies.
Trains and educates patients and family members or others support and documents understanding of treatment plan and goals.
QUALIFICATIONS
Bachelor's Degree- Physical Therapy - Required
1 Year Physical Medicine/Rehabilitation Experience- Required
1 Year Home Health Experience - Preferred
Physical Therapist (PT) State Licensing Board - Required
Cardiopulmonary Resuscitation (CPR)- American Heart Association - Required
Driver's License - Required
Auto Insurance - Required
PTA
Once a Physical Therapist has evaluated a patient and determined the course of action, the Physical Therapist Assistant often takes charge. This job requires someone who is comfortable following and giving orders. Someone with a careful eye who can modify exercises as needed and make sure the patient is doing them correctly. It’s a hands-on, patient-focused job with big rewards as you watch your patients increase function and mobility.
WHAT YOU WILL DO
Provides physical therapy, including selection and use of assistive equipment.
Reports patient progress and collaborates with evaluating therapist to implement therapy plan of care.
Performs comprehensive physical therapy treatment and related follow-up services including discharge planning.
Documents and submits patient reports within the established timeframe.
Provides physical therapy according to standards of care defined by the state practice act and other professional and governing agencies.
Collaborates with patient and other healthcare personnel or outside agencies regarding coordination of care.
Trains and educates patients and family members or other support and documents understanding of treatment plan and goals.
Provides services and treatments requiring physical therapy skills according to the written plan of care and the signed plan of treatment.
QUALIFICATIONS
Associate's Degree- Accredited Physical Therapy Assistant Program- Required
Physical Therapy Assistant (PTA)- State Licensing Board- Required
Cardiopulmonary Resuscitation (CPR)- Amer
