PRIMARY FUNCTION:
Primary Function:
Provision of psychosocial assessment, intervention, resource support, and care navigation for older adults and their caregivers. This role works as a part of the interprofessional Center for Healthy Aging whose mission is to improve the lives of older people by partnering with all patients, all families, and all communities., and vision is Model excellence in collaboration, inclusion and person-centered healthcare
Essential Functions
• Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
- Conduct comprehensive psychosocial assessment of older adults and their supports to evaluate social, emotional, functional and caregiving needs.
- Develops and implements care plans to address identified needs alongside interprofessional healthcare team.
- Collaborates with the interprofessional healthcare team for creation and provision of care plans, engages in team meetings to promote care quality, and provides education to interprofessional teams in areas of community resources and psychosocial issues impacting older adults
- Provides counseling, psychoeducation, and crisis intervention to older adults and their caregivers to enhance coping, address age related concerns, mental health needs, and caregiver distress
- Part of an interprofessional home based care team. Home-based visits based upon need.
- Advocates on behalf of the patients/families to enable them to obtain necessary services.
Competencies (Knowledge, Skills, and Abilities Required):
Knowledge of Social Work Theory and understanding and acceptance of Social Work Code of Ethics.
Experience with and dedication to pursuing cultural awareness with diverse patient populations.
Ability to function as a member of an interprofessional team.
Minimum Education, License/Certification, Experience Requirements:
LCSW preferred or pending.
LCSW or LSW by the State of Pennsylvania.
Master’s degree in social work from an accredited graduate school of Social Work.
Two years’ experience working in healthcare preferred.
Experience with older adults preferred.
Training time on the job is six months to one year.
