Position Summary
A Team Leader provides oversight and supervision to staff and is responsible for coordinating comprehensive services for individuals their program supports. The Team Leader works closely with program leadership, as well as clinical, programmatic staff and employment specialists to ensure that each individual supported is empowered to live the life that they want to live. A commitment to the RHD values should be demonstrated as job duties are performed.
Essential Duties and Functions
Staff Supervision
· Provides first-line assistance to direct care staff within their program through coaching, mentoring and quality service delivery of assigned individuals.
· Partner with the Program Manager to organize, schedule and distribute work on assigned caseload including attendance taking at assigned site.
· Keeps personnel informed of pertinent policies and procedures affecting the program and/or their jobs.
· Administers policies and procedures as established by Resources for Human Development
· Implementation of frequency and duration as described the Individual Service Plan for each individual.
· Identify, staff and coordinate all community inclusion events.
Direct Care
- Maintain direct service case load.
- Coordination and oversight of medical appointments, including attending appointments if necessary. Communication with nurses and doctors about consumers’ medical needs as appropriate.
- Maintains positive communication and relationships with family members, guardians, staff, and Regional Office personnel about policies, issues, and/or concerns.
Administration
- Actively participates in the person-centered planning process, including but not limited to:
- attending all planning meetings,
- providing input,
- coordinating the development of training plans,
- ensuring all necessary documentation is completed,
- reviewing progress notes and daily notes,
- monitoring for the implementation of the plan,
- completing monthly reviews, and
- communication with service coordinators.
- Prepare and submit applicable reports and documentation related to individuals supported by their program.
- Ensure compliance with all regulatory bodies.
- Implement and complete continuous quality improvement activities.
- Other duties as assigned
Qualifications
- Bachelor’s Degree in a Human Services field, including but not limited to sociology, special education, rehabilitation, nursing, counseling, and psychology OR
- An associate degree with one (1) years’ experience working to support individuals with intellectual disabilities OR
- A High School Diploma with threee (3) years’ experience.
- Valid Driver’s License
- IDEATE requires all Team Leaders to have their ACRE, CESP or MG&A certification upon accepting the role.
