01/12/20262000 16th St, Denver, Colorado, 80202-5117, United States of AmericaDaVita’s National Transplant Team is seeking a Program Manager, Transplant to focus on developing and strengthening relationships with transplant centers, establishing DaVita as the "Transplant Partner of choice."
This vital role is central to our mission of improving patient access and equity in kidney care, as transplantation offers the best quality of life for most patients. The Program Manager will drive key initiatives aimed at fostering collaboration, enhancing communication, and standardizing engagement with our transplant center partners to ultimately improve patient outcomes.
The essential duties of this position revolve around strategic partnership and program development. This includes serving as a primary point of contact for transplant center leaders, organizing regular leadership calls, and expanding the Transplant Care Coordination (TCC) Memorandum of Understanding (MOU). The manager will also implement standardized processes for engagement, such as patient pipeline calls, and support the growth of the Transplant Liaison program to provide patient support and strengthen partnerships.
Responsibilities:
In addition to the above, key responsibilities include:
Develop and maintain transplant centers' partnerships, facilitating efficient and effective workflows between internal and external stakeholders
Support seamless care coordination by working with internal teams and promoting best practices for tracking patient journeys, such as facilitating EHR access.
Collaborate with transplant centers to break down barriers to transplantation and support outreach initiatives like "Lobby Days.”

Qualifications:
A bachelor’s degree is required; a master's is preferred
1-3 years of transplant experience highly preferred, including familiarity with transplant center processes and operations, understanding of the patient transplant journey, and understanding of the transplant landscape
1-3 years of experience in healthcare business development and stakeholder engagement preferred
Microsoft Excel and PowerPoint proficiency required; extensive experience developing PowerPoint presentations and presenting to leadership, and experience with Excel data manipulation preferred
Advanced critical thinking, excellent interpersonal skills, strong work ethic, creative mindset, adept communication skills, attention to detail, design-mindedness, flexibility with change, comfort with ambiguity, efficient time management, technical aptitude, and business acumen
