General Position Description: The Medical Behavioral health clinician plays a pivotal role in facilitating integrated care coordination for individuals of all ages within our organization, ensuring seamless collaboration between our behavioral health programs and Medical programs.


Essential Duties and Responsibilities:

  • Assesses the member’s behavioral health needs and eligibility for services. Assists staff in developing and monitoring member service plans.
  • Provides comprehensive intakes, assessments, annual assessments, and offers therapeutic and support services. Screens, evaluates, and schedules referred members who may need behavioral health services. 
  • Coordinates communication between the member and other providers or community agencies to ensure coordination of care. This includes assisting with scheduling appointments, attending telemed appointments, arranging transportation, conducting CFTs and ARTS, and coordinating with the hospital for d/c appointments. 
  • Provides rehabilitation services that include living skills training, counseling, and supported employment services to enrolled members.
  • Provides support services like case management, personal assistance, family support, peer support, respite care, and transportation services to enrolled members.
  • Implements health initiatives and brief interventions to support health and wellbeing for members
  • Performs crisis intervention and solution-focused interventions with members in crisis by working closely with BHMP and clinical team in developing safety plans
  • Conducts home visits for general assessment and to support member clinical needs
  • Assists members in obtaining health screening and maintenance
  • Spends time with community stakeholders, may spend some time stationed at the Crisis Response Center in order to collaborate and support members in crisis
  • Conducts Intakes based on the needs of the assigned work location.
  • Maintains EBF productivity, timelines and quality standards set forth by EBF.
  • Provides complete documentation and reporting of member behavioral health services in accordance with agency and funding requirement. Maintains member records in accordance with agency and funding source requirements.
  • Works closely with behavioral health medical professionals (BHMPs) and other clinical team members to support medical service recommendations and needs. 
  • Participates in the process of ongoing professional development and meetings. Maintains comprehensive communication among all contacts within the agency, members, and external agencies. Models professional standards and ethics in accordance with EBF Code of Conduct, Strategic Plan, Mission, and Vision. 
  • Performs other duties as assigned.

Requirements

Education and Experience:

Behavioral Health Technician (BHT): 

  • High School Diploma/GED with 1 year of behavioral health or related work/volunteer experience; or
  • One year of college; or
  • Four years of full-time experience in behavioral health or related services
  • Possess a Peer/Family Support certificate.

Behavioral Health Professional (BHP)

  • Associate’s license in Social Work, Counseling, Marriage and Family, or Substance Abuse recognized by the Arizona Board of Behavioral Health Examiners. 

Regulatory:

  • Must be at least 21 years of age. 
  • Current, unrestricted AZ driver’s license with no more than two (2) minor moving violations or one (1) accident within the past three (3 years). Three (3) years of driving experience required.
  • Must be able to pass a criminal background check.
  • Ability to obtain and maintain an AZ DPS Level 1 fingerprint clea
