A high quality geriatric practice in Centerville, Ohio is looking for a highly motivated NP/PA with 1 to 2 years experience to work independently at nursing homes as part of a physician NP/PA team.
Responsibilities include: coordinating transition of care of skilled patients from the NH to home and caring for the long term residents in a nursing home or assisted living setting.

Essential Duties and Responsibilities:

- Assist the Physician in treating patients.
- Evaluate patients, take history, and perform pertinent physical examination.
- Provide post-surgical would care and follow up.
- Coordinate care of patients (testing and preparation for procedures)
- Record and ensure availability and completeness of documentation, reports, and imaging examinations.
- Provide patient education, counseling, and instruction.
- Facilitate supervising Physician referral of patients to the appropriate health care facilities, agencies, or other resources of the community or other physicians.
- Dictate progress notes, history, and physical, and discharge summary.
- Administer and/or order medications.
- Initiate emergency measures on patients as required.
- Perform chart review.
- All other specific tasks delegated by supervising Physician in accordance with the Physician Assistant’s knowledge and expectance developed under the supervising Physician.
- Adhere to office policies and procedures.
Minimum Qualifications Required:
- Hold a current license in good standing.
- Maintain certification in accordance with Department of Regulations
- Able to perform health care tasks efficiently and effectively consistent with patient’s health and welfare.
- Excellent interpersonal skills
- Ability to work with minimum supervision.
- Good diagnostic capabilities
Education and Experience Required:
- Graduate of an approved program or its equivalent
- Meets the standards set by the Board of Medicine
- Experience in Internal Medicine/Geriatrics preferred, but not required.
