Home Care RN & LPN | Flowery Branch, GA

 

Key Responsibilities:

• Assess wounds (pressure ulcers, surgical/incision sites, chronic wounds) and create individualized care plans
• Perform wound dressing, debridement, infection control, and skin care as needed.
• Monitor wound healing and report changes to primary clinicians
• Maintain clear accurate documentation in EMR per agency and regulatory standards

Qualifications:

• Active RN and/or LPN Licenses in the state of Georgia.
• Minimum of 1-2 years nursing experience, wound care preferred.
• Home health, Hospice, and/or home care experience a plus. 
  
  

Proven wound care competence (dressing changes, debridement, and infection control)
Good understanding of home health operations.
Must have reliable transportation, valid driver’s license, and pass MVR check.
Positive attitude and strong work ethic
