Registered Nurse, Home Health - Full Time (Wound Care)

We are looking for a compassionate and reliable WOCN to provide care and treatment to ill, injured, and recovering adults in our patient-centered care model. In this role you will make clinical assessments of situations and manage treatment autonomously. You will be a subject matter expert and be asked to consult in person or virtually on many different types of wounds in our geographical service area.
This position will have an opportunity to partially work from home on a hybrid basis 80/20.
Job Responsibilities:
- You will provide care to patients with wounds, ostomies, urostomies, fistulas or skin conditions. You will demonstrate the ability to manage critical situations and make care-based decisions.
- Completes wound care assessments, develops appropriate plans of care, and initiates wound care regimens.
- Maintains a current list of patients with skin care needs.
- Ensures that all photos are taken on admission and on a weekly basis.
- Assists nurses during a co-visit with complex wound treatments.
- Updates wound protocols according to evidence-based literature.
- Networks with hospital staff and peer-level staff to share best practices.
- Respects and considers patient/family preferences, goals and privacy when planning/delivering care.
- Builds strong relationships with coworkers and other staff, recognizing individual talents and perspectives.
- Assumes full patient assignment if deemed necessary.
- Provides clinical education and training to enhance wound care skills
- Coordinates weekly wound visits and stages pressure injuries. Takes weekly photo when staging pressure injuries.
- You will act as an advocate for patient safety.
- You will provide general nursing care while effectively communicating pertinent clinical information to physicians, nurses and other support staff.
- You will ensure effective care and use of time, supplies, and equipment.
- You will act as a resource for all team members, managers and collaborate with physicians in the community.
- You will educate patients regarding their medications, procedure and treatments, providing patients and their families with support throughout their stay.
- You will accurately complete documentation on patient’s record.
·Job Qualifications:
- Registered Nurse with a State of Florida license.
- WOCN certification required.
- Member of the Wound Ostomy and Continence Nurses Society preferred.
- Basic Life Support (BLS) certification required.
- Home Health Experience preferred
