Job Title: Care Navigation Director 

Location: 3935 Sunnyside Drive Harrisonburg VA 22801. Corson Clinic & Highlands Clinic.

Hours: Full Time, Exempt.


Position Overview & General Responsibilities:

The primary responsibilities of the Care Navigation Director are to coordinate activities of clinic services – i.e. medical and specialists appointments. Provide oversight of employee health including new employee health assessments and compliance testing as well as evaluations of job-related injuries. In addition, provide clinical services for Independent Living residents through management of 2 clinics on campus, Corson and Highlands. Implement new programs to benefit the wellness and welfare of residents. Maintain clinic budget through revenue and expenses. Provide education to team regarding new initiatives and clinical compliance. Manage scheduling for both clinics including 24/7 clinic in Highlands and on call nurse. Provide wellness and emergency information to residents and employees with governmental direction. Complete new resident assessments and screenings as well as assist with discussion and logistics for internal transitions. Contribute to an interdisciplinary team to discuss resident status and outcomes. Manage campus events such as vaccine clinics, wellness fairs, and additional community supports.

  • Aid physicians and specialists during office hours at the clinic. Follow through with orders for residents and monitors resident charts from all areas.
  • Provide follow-up on workers’ compensation claims along with Human Resources Director. – Provide good organizational skills to maintain OSHA regulations.
  • Ensure assessments for new employees are completed. – Administer PPD, drug screen and initial health assessments upon hire and/or as required by corporate policies and procedures. Make Human Resources Director aware of any health concerns that may hinder employee from performing the essential functions of their job.
  • Does urine checks, EKG’s, glucose blood tests and phlebotomies as needed for residents. – Use proper procedure and report immediately to appropriate staff.
  • Review medical certifications for each prospective resident for independent living. – Make appropriate recommendations. Tour prospective residents in clinic, providing overview of services.
  • Provide first aid treatment for residents and employees. – Provide immediate treatment and decide for further treatment if necessary for employees and independent residents.
  • In cases of blood and bodily fluid exposure to employees, perform follow-up and complete necessary paperwork. – Follow protocol for all exposures according to Sunnyside policy and procedures.
  • Analyze and comprehend thermometer, pulse oximeter, blood pressure and blood glucose readings. – Accurately interpret test results.
  • Provide care to residents who require ear lavage / cleanings as requested. – Use proper procedure in cleaning residents’ ears.
  • Perform telephone pacemaker checks for all residents. – Coordinate with cardiology office and perform checks on each resident.
  • Operate clinic equipment, i.e. EKG machine, centrifuge, pacemaker for lab testing as well as all equipment normally found in physicians’ office. – Use equipment appropriately to perform job duties.
  • Coordinate annual influenza and additional vaccine injections for employees and residents as well as annual tuberculosis screening for employees. – Assure that dates are published for employees and residents prior to the administration dates. Assure that adequate supplies are available for each injection series. Administer injections safely and accurately according to nursing standards. Refer all positive TB tests for further evaluation.
  • Ensure that all medical independent contractors have met the requirements according to corporate policy ADM.003. Obtain all documentation for extra duty sitters and ensure documentation is current.
  • Oversee all Highlands Clinic staff and provision of services. – Ensure services provided to residents are of good quality.
  • Maintain annual budget for 2 clinics with monthly reporting and auditing.
