Case Manager I - Family Partner

Are you passionate about making a difference in the lives of youth and families? As a Family Partner, you will provide one-on-one support to parents and caregivers, helping them navigate child-serving systems and improve the functioning of youth in the community. You’ll collaborate with clinical teams, offer education, advocacy, and resource development, and empower families to achieve better outcomes.

Responsibilities

• Support families with care planning, coaching, and advocacy.
• Assist parents in navigating systems such as education, mental health, and juvenile justice.
• Provide high-quality, client-centered care and maintain professional standards.
• Ensure timely documentation and effective communication.
• Work closely with interdisciplinary teams and attend required meetings and trainings.

Qualifications

• Lived experience as a guardian or caregiver of a youth with disabilities or mental health challenges is mandatory.
• Experience in youth/family case work or wraparound care (preferred).
• Knowledge of mental health, child welfare, or juvenile justice systems is a plus.
• Strong communication, organization, and collaboration skills.
• Valid driver’s license and reliable transportation required.
