Responsibilities include evaluating patient health status, identifying medical concerns, and developing care plans in collaboration with providers, external organizations, and patients. The role also involves conducting nurse visits, performing in person and telephonic triage, delivering in-person and telephonic patient education, providing wound care, supporting providers, and addressing routine patient inquiries.
Minimum Education Required: Graduate of an approved school of nursing, currently licensed as a Registered Nurse in the State of WI.
Credential/Licensure Required: BLS (Can obtain on the job).
Shift: Working 40 hours a week at the Sturgeon Bay Clinic location.
