About the role

The LPN Clinic is responsible for providing patient care following established standards and practices.  Assists physician, nurse, and staff with the daily coordination of patient care and patient access.  Coordinates patient flow,  schedules appointments, procedures, tests and referrals. May serve as a preceptor for clinical staff. Relies on guidelines to accomplish tasks.  Relies on some experience and judgment to complete job.  Works under general supervision. May lead and direct the work of others within scope of practice.

 

The Medical Assistant assists in examination and treatment of patients under the direction of a physician. Interviews patients, measures vital signs (i.e., pulse rate, temperature, blood pressure, weight, and height), and records information on patients' charts. May serve as a preceptor for clinical staff. May be required to draw and collect blood samples from patients and prepare specimens for laboratory analysis. Coordinates patient flow, schedules appointments, procedures, tests and referrals. Works under close supervision.

LPN

1. Patient Care:

  1.    Observes, records and reports (to physician) patient's condition and reaction to drugs and/or treatments. Instructs patient/family regarding diagnostic procedures, medications, nutrition and health/wellness issues.
  2. Performs selected nursing duties, including documenting patient history, vital signs, immunizations and allergies. Prepares, stocks and cleans exam room and maintains supplies and equipment for treatment.

2. Coordination of Care:

  1. Provides complex and basic nursing care measures to all age groups. Assists physician with patient care, clerical, environmental and organizational tasks.
  2. Responsible for appointment and referral scheduling, chart management, providing information and instructions to patients, and coordinating patient flow.
  3. Prepares exam room and assists physician during examination, treatment and testing of patient.

3. Quality:

  1. Administers prescribed medications/treatments in accordance with nursing standards. May perform phlebotomy and EKG's and assist with flex sigmoidoscopies.
  2. Observes and adheres to all departmental and hospital policies and procedures, and follows all safety, quality assurance, and infection control standards.
  3. Utilizes all equipment, supplies, facilities, and resources in a prudent and efficient manner in order to ensure efficient departmental operations and the provision of high-quality health care services.
  4. Observes and adheres to quality standards established by the department. Participates in continuous quality improvement programs in order to foster the provision of quality health care services by the department and hospital and ensures that all services are provided with compassion, understanding, and respect for others.

Medical Assistant

1.   Patient Care
a.   Properly identifies patient by requesting patient name, DOB prior to any patient interactions.
b.   Ensures confidentiality of protected patient health information by utilizing safe business practices that support HIPAA regulations.
c.   Completes and documents patient visit notes accurately which may include but not limited to the chief complaint, past medical and surgical history, family history, vital signs, immunizations, allergies, medications, nutrition and health and wellness issues as directed by physician practice and organizational processes.
d.   Accurately and safely administers and documents waived testing under the direct supervision of physician.
e.   Prepares, stocks, and cleans exam rooms and maintains supplies and equipment for patient care.
f.   Prepares and administers medications according to physician/physician assistant orders and organizational policies and procedures.
g.   Assists the physician with minor surgical procedures and follows appropriate procedures for obtaining handling and processing specimens and cultures.
h.   Provides basic clinical care measures to all age groups per clinical skills checklist.


2.   Coordination of Care
a.   Coordinates patient flow to ensure physician efficiency includes managing the patient schedule and wait times.
b.   Instructs patient and/or family regarding diagnostic procedures, medications, nutrition and health/wellness issues using pre-approved forms and information.
c.   Retrieves and sends requested patient health information to authorized individuals in a timely fashion in accordance with HIPAA regulations and organizational policies and procedures.
