The Clinical Case Manager assists in the management, development, and oversight of the individual support plan. They ensure the individual support plan is implemented as written and determines progress. The Clinical Case Manager collaborates with the residential team leaders, support coordinators, clinical staff, nurses, and other members of the management team through executing the following duties and responsibilities.Salary:$55,000-$65,000 annually

SUPERVISORY RESPONSIBILITIES:
This position has no supervisory responsibilities.
DUTIES AND RESPONSIBILITIES:
- Assist with coordinating IDT meetings and the ISPs
- Supervises and monitors the implementation of the Individual Support Plan (ISP).
- Ensure a streamlined admission process.
- Responds to incident reports through collaboration with staff to develop root cause analysis, response plans.
- Analyzes and summarizes progress status by determining the program appropriateness and recommending changes as appropriate.
- Communicates regularly with the Support Coordinator and other members of the circle of support if amendments to the ISP are needed.
- Documents all communication.
- Regularly spends time at the location where services are being performed.
- Conducts regularly announced and unannounced visits in residential homes, including each shift, to ensure policy, procedures, and program data are in place and being implemented appropriately.
- Provides coverage if there is a shortages of direct support personnel but, while doing so, continues to be responsible for management duties.
- Assists in on-call rotation as specified by the Administrator, including weekends and holidays.
- Trains and conducts in-services for direct care staff as needed.
- Conducts internal peer reviews, observations, and other internal QA assessments.
- Conducts CQL's Personal Outcome Measures
- Supports key components of the person's health record for accuracy and timeliness.
- All other duties as assigned.
MINIMUM QUALIFICATIONS:
- Bachelor’s degree in Social Work, Social Services, Human Services or Special Education required; Master’s degree a plus
- Minimum 2 years experience working with persons with intellectual and/or developmental disabilities required; 1-year experience if paired with Master’s degree
- Working knowledge of applicable regulations that apply to our service offerings
- Valid driver’s license, auto insurance, and reliable transportation
- Supervisory/management experience and experience writing person-centered (individualized service) plans preferred
- Must be at least 18 years of age.
PHYSICAL DEMANDS AND WORK ENVIRONMENT:
- Regularly required to lift 30 lbs.
- Must be able to lift a minimum of 15 lbs.
- Must be able to pull minimum of 30 lbs.
- Must be able to squat, kneel, crawl, crouch, climb, and stoop.
- Must be able to regularly use hands to finger, handle, or feel objects, tools, or controls.
- Required to regularly stand and walk.
- Must be able to demonstrate proficiency in CPR from the floor level requiring to work on hands, knees, bending, standing and lifting.
- Vision requirements include close vision, distance vision, and peripheral vision.
- Must be able to talk and hear.
