Certified Home Health Aide, Home Health (per diem)

Assisted is looking for a per diem Certified Home Health Aide (HHA) for Home Health, for our Los Angeles branch to pick up about 5-10 visits per week.<br />

You must have a Home Health Aide Certification through the California Department of Public Health.<br />

<b>The impact you will make:</b>

<ul>
<li>Assist patient with personal hygiene, elimination, / nutritional support, ambulation and exercise according to the patient’s needs and as specified on the CHHA Assignment.</li>
<li>Ensure quality and safe delivery of CHHA services per written plan of care.</li>
<li>Communicate with RN, Rehab Case Manager or Clinical Supervisor on a regular basis regarding changes in the patient’s condition and/or needs.</li>

<b>Who you are: </b>

<ul>
<li>Completion of an approved training program pursuant to Federal/State law.</li>
<li>Current, active California CHHA certificate with criminal background check.</li>
<li>Current CPR, health clearance and completed skills assessment.</li>
