What You’ll Do:

• Provide skilled nursing care (medications, wound care, vital signs) in patients’ homes
• Collaborate with the healthcare team to create personalized care plans
• Educate patients and families on care and treatment
• Maintain accurate patient records and communicate effectively with all parties

What We’re Looking For:

• RN license in IL
• 2+ years of nursing experience (home health or hospice preferred)
• BLS/CPR certified
• Strong clinical skills and compassionate care approach
• Ability to work independently and reliability
