This role supports patients in remaining in their home setting whenever possible while receiving high-quality, compassionate care.</p> <hr> <h3><strong>Qualifications</strong></h3> <ul> <li>Minimum of one year of clinical experience as a licensed Physical Therapist required.</li> <li>Graduate of an accredited physical therapy program.</li> <li>Home health or hospice experience strongly preferred; or an equivalent combination of education and experience that provides the necessary skills to perform the role.</li> <li>Ability to work independently and manage time effectively in a field-based environment.</li> <li>Strong clinical assessment, problem-solving, and organizational skills.</li> <li>Excellent interpersonal and communication skills.</li> <li>Proficiency with electronic medical records (EMR) systems preferred.</li> <li>Valid driver’s license and reliable transportation required.</li> </ul> <p>#INDOTH</p> </div><div class="content-pay-transparency"><div class="pay-input"><div class="description"><p>The Pay Range listed is the anticipated hiring range of this position. &nbsp;Precise pay rates are determined through consideration of one or more of the following:&nbsp; education, training, relevant experience and work location.&nbsp; There are some cases in which the pay rate could exceed the range listed on this job post.&nbsp; Care Dimensions also
