The Hospice Intake Coordinator facilitates the provision of intake and referral services for hospice inquiries to assist with meeting the needs of the patient and caregivers. They use a holistic approach, keeping with the patient and caregiver’s beliefs and goals of care to ensure adequate coordination and transition to hospice services. The Hospice Intake Coordinator coordinates and collaborates with internal and external sources to ensure the Hospice IDG Team, patient, caregivers, providers, referral sources and community resources to work toward plan of care goals. The Hospice Intake Coordinator serves as a resource regarding all hospice services, hospice guidelines, regulations and hospice philosophy for patients, caregivers, providers, medical director, referral sources, community sources and Hospice IDG members. The Hospice Intake Coordinator participates in a variety of different types of patient visits as the department requires. The Hospice Intake Coordinator performs administrative on-call responsibilities, holiday staffing and participates in emergency preparedness to meet the department needs. Other Items of Note:
Preferred Education:
Bachelor’s Degree in Nursing
Required Skills:
Nursing skills as defined by generally accepted standards of practice. Basic knowledge and skills to care for the clinical needs of the population served. Professional and efficient verbal and written communication skills. Good interpersonal skills. Ability to multi-task, basic organizational and time management skills. Basic computer skills. Knowledge of general office and administrative practices and procedures. Knowledge of how to utilize office equipment (copy machine, fax machine, etc.) Ability to be open, sensitive, flexible. Must have demonstrated experience in working with patients and caregivers dealing with life-altering and terminal illness and end of life. Must be capable of prioritizing and handling multiple tasks of critical importance simultaneously.
Preferred Skills:
Home Care Home Base experience; Experience with smart phones and smart devices; Experience with Microsoft Office; Experience with Meditech; Experience with Athena Required Licensure/Registration/Certification:
Current Texas state license in appropriate discipline; Current American Heart Association CPR; Current Texas state driver’s license; Current auto insurance. Must have reliable transportation.
Preferred Licensure/Registration/Certification:
Hospice and Palliative Care certified; Wound Care Certified; Advanced education or certification in area of specialty.
Required Experience:
Two years’ experience in healthcare setting in respective discipline
