About the role:

Accompany Health Pharmacists serve a key role in our Integrated Care Teams, which also include Advanced Practice Clinicians, Physicians, Community Health Workers, Patient Experience Navigators, RNs, Social Workers, Behavioral Health Clinicians, and Psychiatrists. Together this team is responsible for providing and coordinating holistic, patient-centered care for an intimate panel of patients with complex medical, behavioral health, and social needs.
As an Accompany Health Pharmacist, you will be responsible for ensuring evidence-based, safe, and cost-effective drug therapy for our patients in collaboration with the Integrated Care Team and external providers. You will provide direct and consultative care to our members and their caregivers in a Comprehensive Medication Management (CMM) model to identify and resolve medication therapy problems while helping design individualized therapeutic regimens for optimal management of chronic disease states (e.g. diabetes, CHF, COPD, CKD/ESRD). You will lead processes to ensure appropriate access, affordability and adherence to medication regimens. Together, your efforts will drive better individual patient outcomes, improve quality performance, and prevent avoidable ED visits and hospitalizations.

Responsibilities will include:
- HEDIS Quality Measures:
- Support efforts to improve HEDIS medication adherence and other pharmacy quality measures, including but not limited to adherence to antihypertensive, cholesterol-lowering, and diabetes medications
- Collaborate with cross-functional teams to promote adherence to prescribed therapies
- Provide day to day oversight of pharmacy program workflows to support the pharmacy team in implementing targeted interventions to improve adherence rates
- Patient Engagement:
- Directly engage patients, as needed, to address barriers to adherence, provide patient education, and ensure understanding of medication regimens
- Work with the pharmacy team and patients to identify and resolve issues related to medication adherence, including cost, side effects, and complex dosing schedules
- Utilize motivational interviewing techniques to encourage patients to adhere to their prescribed medication regimens
- Data Analysis & Monitoring:
- Utilize available data to identify patients at risk of non-adherence and support the pharmacy and clinical teams in developing action plans for complex patients
- Assist with monitoring and tracking HEDIS performance metrics and quality opportunities to assess the effectiveness of interventions and program workflows
- Clinical Support & Training:
- Provide clinical support for medication opportunities, including drug interactions, deprescribing, formulary alignment and prior authorizations, drug selection, and other clinical needs
- Participate in clinical and non-clinical staff education related to pharmacy services, optimal medication adherence, and best practice prescribing
- Assist with development of medication management and disease state-specific clinical pathways/protocols
- Ensure timely and appropriate documentation within our electronic medical record as well as our custom-built platforms that support care for our patients
- Participate in interviewing and training pharmacy technicians and fellow clinical pharmacists
- Roles and responsibilities may evolve as our care model develops
