This is a Hybrid role with a commute to our Cleveland FreedomCare office 2x/month.

Department & Position Overview: 

Personal Care Services is a caregiving arrangement in which a patient has a principal caregiver who provides daily support based on the patient's daily care needs. The goal of this service is to provide necessary care while fostering and emphasizing the patient’s independence in the patient’s home that will provide them with a range of care options as their needs evolve. The service is designed to provide options for alternative long-term care to persons who meet nursing facility-level care needs while preserving the dignity, self-respect, and privacy of the patient. 

In this position, as a Licensed Practical Nurse, you will perform clinical evaluations in the home of the Personal Care Services member to determine the level of service to qualify for the program. You will also conduct monthly visits with members who are On Care with FreedomCare to ensure they are receiving quality service and address any changes in their condition.

Every Day You Will:

· Provides support to the caregiver and member, and reviews services according to scope of Personal Care Services.

· Respects client and employee rights and privacy, ensure the security of protected information, practices in an ethical manner and is compliant to the agency’s compliance program and privacy policy. ●

· Follows the FreedomCare’s clinical policy and procedures.

· Assesses and regularly reassesses the client’s needs and their PCP summary form, treatment and education and communicates any changes to the member’s PCP.

· Develops, implements, evaluates, revises, and provides input to the client’s plan of care to include the physician’s plan of treatment, and problem list. Provides instruction of care to Caregiver according to the plan of care.

· Completes all client documentation, to include written records, reports, and notes in accordance with agency policy and procedure following client visits or contacts in an accurate, timely, complete, concise and legible manner.

· Actively coordinates the care of the client through communication and written documentation to the physician, RN and Program Director, other disciplines involved in the client’s care and community resources.

· Interprets the philosophy and objectives of the agency to clients, families, and others.

· Performs other job-related duties as may be required or directed by the supervisor.

Ideal Candidate Will Possess:

· Licensed Practical Nurse licensed by Ohio Board of Nursing. ·

· Minimum of 2 years clinical experience, 1 year of experience with elders or adults with disabilities

· Bilingual in Spanish is a plus ·

· Experience with CRM (Salesforce is a plus) ·

· Valid driver’s license with an insured vehicle for travel within the state

·  Understanding and comfort being a stranger in someone's home for field visits
