Join our team and make a positive impact on the lives of others! We are looking for a Reassessment RN for our Licensed Home Care team.

Department & Position Overview:

We are looking for compassionate, empathetic Reassessment Nurses to assess our patients current needs.

Every Day You Will:

• Assess eligibility of patients in need of managed long-term care
• Conduct UAS (Uniform Assessment System)  assessment at skilled nursing facilities, or in a client's home, according to state guidelines
• Consult with health care providers and review medical records and medications to support assessment outcomes
• Conduct needs assessment by utilizing UAS-NY / DOH tools to determine a patient's qualification for Long Term Care Services

Ideal Candidate Will Possess:

• At least an Associates Degree and current NYS License that is in good standing
• 3+ years Nursing experience
• Ability to travel within the 5 boroughs

• Current knowledge of state and federal home health regulations and community service systems
• Strong experience using a Uniform Assessment System (UAS); proficient in Microsoft (Word, Excel, Outlook)
• Excellent verbal and written communication, organizational and time management skills
• Strong critical thinking and problem solving skills

Nice-to-Haves:

• Bilingual in English and one of the following languages: Spanish, Russian, Chinese, Korean and Bengali 
• Experience in Home Care, Geriatrics and Case Management; field experience assessing patient care
