The Western Tidewater Health District is looking for a Public Health Nurse to join the Nurse Family Partnership (NFP) Program team. This position provides home visiting services and case management to families in the City of Suffolk, City of Franklin, Isle of Wight, and Southampton Counties. A Nurse Family Partnership Nurse provides the support, advice, and information needed to have a healthy pregnancy, a healthy baby, and be a great mom.

The Nurse-Family Partnership (NFP) Nurse Home Visitor is responsible for providing comprehensive nursing services to women and their families eligible for the NFP Program. Nurse Home Visitors (NHV) are pivotal in transforming the lives of vulnerable babies, mothers and families by implementing and maintaining the fidelity of the NFP model. Our goal is that all children are healthy, families thrive, communities prosper, and the cycle of poverty is broken. Nurse home visitors report to the NFP Nurse Supervisor.

This position independently provides professional nursing care to patients in clinics, homes and schools and other community or congregate care settings. Performs nursing assessments, may perform lab tests, take measurements, develop, and implement care plans, including the administration of medication and case management. Participates in public health emergencies and community health assessments. Actions performed at this level require licensure as a RN by state code. May supervise support staff.

Essential Responsibilities and Duties

Provides home visits to women and their families eligible for the NFP program.
Adheres to nursing process and the NFP model of home visitation.
Carries a caseload of up to 25 clients.

VDH requires all Health-Care Personnel (HCP) to provide documentation of immunization or evidence of immunity to the following: hepatitis B, measles, mumps, rubella, and varicella; a one-time dose of tetanus, diphtheria, and pertussis (Tdap) vaccine regardless of the interval since the last dose of tetanus-diphtheria (Td) vaccine was administered (Td or Tdap boosters shall be received every 10 years); and the current year's seasonal influenza vaccination.

Minimum Qualifications

• Communicates verbally and in writing, to analyze problems, situations and suggest solutions, leadership, follow directions and establish priorities

• Skills in physical examination, health assessment, and interviewing and counseling technique

• Ability to function independently, prioritize and organize work, solve problems, adapt to change, function as a team member and relate to the public

• Knowledge of public health concepts, principles of public health nursing, principles of epidemiology; immunizations; basic nursing science, medical terminology, case management

• May travel and some weekend and/or evening hours may be required

• Must be certified in Basic Life Support (BLS)/CPR

Must be a Registered Nurse.

Additional Considerations

Experience in a public health setting or one related to disease prevention and health promotion

Experience in the maternal/child setting and/or home visiting is preferred.
