Home Health Aide – HHA Certificate required

A supportive non-profit is Seeking Home Health Aide (CAN and HHA) for hospice that is committed to providing exemplary, culturally competent care for their patients. Provide care with sensitivity and compassion and create a safe and supportive environment. Need to have a Home Health Aide certificate. 

Responsibilities: 

• To coordinate and provide care to our hospice patients
• To calm, soothe and empathize with the strong emotions our patients and families are experiencing
• To participate with a team of healthcare professionals in superior level of care for each patient
• To address personal care needs and promote patients’ comfort needs 
• Collaborate on cases, involvement in interdisciplinary meetings, training and development and other organizational activities
• Assist with observing the patient along with documenting care

Requirements

Requirements

· Certified Home Health Aide (HHA) issued by the State Department of Public Health and Certified Nursing Assistant Certificate (CNA) issued by the State Department of Public Health required

· Current BLS/CPR certification

· One year aide experience required and one year of hospice experience preferred 

· Possession of a valid California Driver's License, access to a reliable automobile and minimum car insurance coverage

· Excellent observation, problem solving, verbal and written communication skills

· Demonstrated ability to efficiently use a computer, prior electronic medical record
