Position Summary

This licensed nursing work involves assisting Palliative Care Managers, Clerical Staff and Field clinicians by responding to patient/staff inquiries, coordinating new referrals, coordinating care for current patients and those discharging from an inpatient facility, and obtaining physician's orders. Triage urgent and complex issues that arise in collaboration with the care team, scheduling team, and manager.


Position Specific Responsibilities

  • • Provides assistance to Palliative Care Managers as requested.

    • Provides telephone triage and problem-solves patient issues to ensure the best patient experience.

    • Accepts physicians' telephone orders for patient care services, assesses information and coordinates with Clinical Staff.

    • Reviews clinical documentation and coordinates needed changes with identified Managers and/or Clinical Staff.

    • Responds to general Agency telephone inquires regarding services.

    • Participates in special projects as assigned (for example, quality management, utilization review).

    • Coordinates and prioritizes new patient referrals, including working HCHB work queues.

    • Coordinates care for current patients and those returning after an inpatient stay.

    • Triages and coordinates care for patients needing skilled visits.

    • Runs the after-hours/weekend call report and follow-up with patients and clinicians accordingly.

    • Manages INR results and tracking, and updates the Care Plan accordingly.

    • Assists with other duties as assigned.


Education and/or Experience

  • Required - Graduate of a school of Nursing.
  • Preferred - 1 year Experience in home health, hospice or long-term care setting
  • Preferred - 1 year Experience working as a triage nurse or care coordinator


Skills

  • Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
  • Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
  • Language Skills: Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy.
  • Language Skills: Ability to communicate both orally and in writing.
  • Other Skills and Abilities: Current knowledge of nursing practice and basic knowledge of care in a home setting.
  • Other Skills and Abilities: Knowledge of telephone etiquette.
  • Other Skills and Abilities: Ability to make independent clinical decisions based upon knowledge of the Agency, training and experience.
  • Other Skills and Abilities: Ability to coordinate effective communication with physicians and office staff.
  • Other Skills and Abilities: Thorough knowledge of Skilled Nursing and Home Health Aide visit patterns.


Certifications, Licenses, and Registrations

  • Required within 30 days of hire: National Provider Identifier (NPI) number
  • Required upon hire: National Provider BLS - American Heart Association (Vendor Managed)
  • WA: Required upon hire: Washington Licensed Practical Nurse (Vendor Managed)


Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
