Position Summary:
Provides direct and indirect patient care as supervised and delegated by a Registered Nurse, for all assigned patients in the in-patient unit. Communicates with physicians, mid-levels, Charge Nurse, Nurse Manager and co-workers, as appropriate about patient needs and clinical condition. Is able to respond quickly and appropriately in emergent situations.
Qualifications:
Education/ Experience: Graduate of an accredited school of nursing. Current Licensed Practical Nurse licensure in Vermont. Current BLS certification required. Complete GCH IV therapy course within one year.
Other Requirements: Computer literate. Experience working with an electronic health record preferred.

Essential Functions Of The Position:
- Provides input to RN in formulating, evaluating and revising the patient’s care plan.
- Implements the nursing and medical plans of care within LPN scope of practice and as delegated by the RN, including appropriate patient teaching.
- Identifies psychosocial needs of patients and family and communicates same to RN and Social Service/Discharge Planner for incorporation into the patient’s care plan.
- Maintains current knowledge of growth and development over the life span. Identifies age specific needs and provides age-appropriate care to all patients served. This includes pediatric through geriatric patient populations.
- Assesses, addresses and reassess pain utilizing appropriate pain management techniques and tools. Educates patient and family regarding pain management.
- Treats patients and their families with respect and dignity, recognizing cultural, ethnic and religious/spiritual needs.
- Maintains current knowledge of medications and their correct administration based on age of the patient and patient’s clinical condition.
- Once IV certified through the GCFHH Vascular Access Training, skills will be maintained and demonstrated annually.
- Understands and applies the five medication rights and reduces the potential for medication errors. Bar Code scans whenever possible – minimum of 80%.
- Complies with Pyxis Medication Dispensing policies and protocols.
- Maintains POC competency and documents per hospital standards and policies.
- Assist Providers with procedures as requested and within scope of practice, demonstrating good technical and manual skills.
- Maintains aseptic technique during treatments and special procedures as required.
- Communicates appropriately and clearly to Charge Nurse, co-workers and Providers.
- § Consults other departments, as appropriate, to provide for an interdisciplinary approach to the patient’s needs. Aware of available support services.
- Is flexible, organized and able to function under stressful situations.
- Performs all aspects of patient care in an environment that optimizes patient safety. Manages and operates all equipment safely and correctly.
- Performs other duties as assigned within LPN scope of practice.
- Takes the initiative to find productive and appropriate ways to keep busy during slow times. Limits personal internet/computer use, reading materials and phone calls to break periods.
