Your Role

The Care Navigator is a member of aptihealth’s Integrated Behavioral Health Team embedded within the acute care settings of our Health System Partners. The Care Navigator is responsible for proactively engaging with patients referred to aptihealth from the Emergency Department or other acute care settings. The Care Navigator will ensure a best-in-class start to the patient’s behavioral healthcare journey by educating on aptihealth services, and facilitating registration, screening, and scheduling the patient into care prior to discharge from the hospital. Our goal as a team is to reduce the barriers and challenges that patients face in their search for care and to facilitate frictionless referral and transitions of care processes.

Responsibilities

  • Work directly with patients and their acute-setting care team to connect them with the behavioral healthcare providers and resources they need
  • Provide appropriate information, education, and explanation of aptihealth services to patients displaying a comprehensive understanding of aptihealth’s mission, values, and care model and the ability to communicate it in a clear, compassionate way
  • Understand how insurance works and be able to effectively verify insurance eligibility and communicate patient cost sharing responsibility
  • Facilitate the beginning of the patient’s behavioral healthcare journey using aptihealth’s system
  • Quickly become proficient in aptihealth technology, tools, and workflows
  • Document outreach and related notes in the aptihealth platform and in the partner EMR, where applicable
  • Troubleshoot issues with patient referrals

Measures of success

  • Meet with key individuals within the hospital to establish and secure a pipeline of patient referrals
  • Ability to prioritize daily workloads to ensure patients are finding the care they need in a timely manner
  • Apply expert knowledge of aptihealth’s platform, workflows and market nuances to assist patients in their journey to care
  • Commitment to meet standardized expectations for Care Navigators through established performance metrics which includes growth and volume benchmarking

Requirements

  • Bachelor's degree preferably in psychology, social work, or other related behavioral health/healthcare disciplines or equivalent experience
  • Experience with representing and promoting care services.
  • 2-4 years of patient facing customer service experience in the healthcare setting, preferably in acute care, behavioral health, or substance use treatment facilities
  • Experience & comfort using technology such as computer telephony, virtual visit technology, and EMR platforms to document patient interactions, register, & schedule patients for appointments
  • Strong interpersonal communication skills, written communication skills, and active listening abilities
  • Display passion and care when interacting with patients’ mental health need
  • Experience working in a team environment and enjoy assisting other team members on tasks, duties, and/or project
  • Passionate about being a trusted partner to transform behavioral healthcare
  • Fluency in foreign languages a plus, particularly Spanish, Polish, Portuguese, Russian, and/or Ukrainian
  • Valid U.S Driver’s License and dependable transportation
