DaVita is pioneering a new approach to primary care for patients with end-stage renal disease (ESKD) directly within our dialysis centers. We're seeking a highly motivated and innovative Nurse Practitioner to be our founding NP, helping us build and shape this groundbreaking program.
It is estimated that 30-50% of patients with renal failure do not receive consistent primary care support, resulting in increased morbidity, hospitalizations, and mortality. This program will address the care barriers kidney patients face:
Bringing care to the patient: Patients receive care in the health setting they spend the most time in: the dialysis unit. No additional patient transportation is required.
Fostering seamless coordination: Proximity to and coordination with other critical caretakers, such as the Nephrologist, dialysis teammates, and integrated care leaders.
The Opportunity:
This Nurse Practitioner will be the first hire. Starting a program requires flexibility and patience, but it affords the opportunity to play a pivotal role in defining the initiative’s clinical direction. You will have direct access to top DaVita leaders to provide feedback on innovation and care redesign. It also means that, pending performance, there will be future leadership opportunities as we scale the program.
Compensation:
The NP will receive a competitive base salary, but given the unique role of being a first hire, additional compensation will be provided for meeting pre-determined clinical milestones. This is an exciting opportunity to meaningfully share in the success of a new, high-impact initiative.
Role Specifics
Location: You will work from a portfolio of 3-8 DaVita dialysis centers within the Chesapeake/Norfolk, VA market.
Hours: Your schedule will revolve around the time patients will come into the dialysis units, primarily between 8:00 AM and 5:00 PM. You will have clinical autonomy to determine which patients to see and during which time. There will be an on-call aspect of the program which will require you to support patients if they call you after hours.
Expectations:
Average 10-12 patient visits per day.
Coordinate patient follow-ups to proactively address patient concerns, including coordinating care with other providers, ordering key labs, and prescribing medication.
Build clinical relationships with nephrologists, dialysis teammates, integrated care teammates, and local PCPs and specialists.
Conduct annual comprehensive health evaluations.
Provide telephonic on-call support for patients as needed.
