Job Description:
Provide high quality, patient centric, cost effective in-patient level of care in the home setting.
• Clinical outcomes are equal to or better than inpatient hospitalization.
• Work in a team environment (Physicians, NPs, RNs) in a virtual Command Center
• Medically Home significantly reduces total medicine expense from avoidable inpatient visits, post-acute care and readmissions.
• Improved patient experience.
• State of the art equipment at Command Center and patients’ home.
• Patients have home infusion, meals/nutrient consults, pharmacy/medical mgmt., mobile imaging, laboratory and DME/oxygen.
• Integrated care with primary care providers, specialists & ancillary departments.
Minimum Qualifications:
Licensure, Certification, Registration:
• BC/BE Internal Medicine/Family Medicine/Geriatrics.
Skills, Knowledge & Abilities:
• Experience as a hospitalist or SNF a plus.
• Committed to providing high quality, evidenced based medicine to a diverse patient population.
• Excellent clinical and communication skills.
• Demonstrated ability to work in a team environment.
