Social Worker MSW - Accountable Care

Responsible for providing case/care coordination and a full range of social work services including psychosocial assessment, treatment planning, utilization management, therapeutic interventions, discharge planning, crisis intervention, referral and resource, and liaison with insurance providers.

Required Qualifications:

• Master's Degree in Social Work from an accredited school
• Coursework/Training: Specific Training in the provision of outpatient community services.
• Coursework/Training: Training in using efficient, focused treatment approaches to patient problems.
• Upon hire: Washington Advanced Social Worker License or
• Upon hire: Washington Advanced Social Worker Associate License or
• Upon hire: Washington Clinical Independent Social Worker License or
• Upon hire: Washington Clinical Independent Social Worker Associate License

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
