Your Role
As an Advanced Practice Provider within our Neuromuscular Program, you will serve as a central member of the care team and primary coordinator for patients with complex neuromuscular diseases.
Key responsibilities include:
- Conduct comprehensive patient evaluations, including symptom management, medication review, functional assessments, and ongoing monitoring
- Develop individualized treatment plans in collaboration with supervising neurologists and interdisciplinary team members
- Coordinate care across multiple specialties and services to ensure seamless patient experiences
- Support advanced care planning and goals-of-care discussions
- Facilitate access to specialty therapies, medications, assistive technologies, and supportive resources
- Serve as a trusted resource for patients and caregivers throughout the continuum of care
Practice Environment
This position is primarily outpatient and includes:
- Multidisciplinary ALS and neuromuscular clinics
- Collaboration with highly specialized neuroscience providers
- Participation in case review conferences and care planning meetings
- Potential travel between clinic locations
- Community outreach and educational opportunities
Requirements

Required
- Master's or Doctoral degree from an accredited Nurse Practitioner or Physician Assistant program
- Current California NP or PA licensure (or ability to obtain)
- National certification
- DEA registration (or eligibility)
- Minimum three years of advanced clinical experience
Preferred
Experience in one or more of the following areas:
- Neurology
- Neuromuscular Medicine
- Rehabilitation Medicine
- Palliative Care
- Complex Chronic Disease Management
- Multidisciplinary Specialty Care Programs
