Street Medicine Kalamazoo Nurse Practitioner

Overview

Western Michigan University Homer Stryker M.D. School of Medicine (WMed) has an opportunity for a Street Medicine Nurse Practioner. Street Medicine Kalamazoo (SMKzoo) provides medical and social services to individuals who are underhoused or rough sleeping in Kalamazoo. We provide access to medical, addiction, behavioral health, harm reduction, and social services with a focus on meeting patients where they are at and decreasing barriers to accessing these services. We run shelter-based clinic and mobile clinic that is parked at fixed locations on a weekly basis. We also provide care at encampments and other locations throughout Kalamazoo County where are patients are residing.

 

SMKzoo is seeking an experience Nurse Practitioner (NP) who is dedicated to providing compassionate, whole-person care to individuals with complex and challenging health conditions. The NP will work with our multidisciplinary team to care for underhoused and rough sleeping residents Kalamazoo County. The successful candidate will have with clinical experience in primary care, psychiatry, and/or addiction medicine and be a strong advocate for vulnerable patients and for systems that improve the health of all/are low barrier.

Responsibilities

• Assessment and treatment of acute and chronic physical and mental health conditions individuals of all ages in community-based settings.
• Perform field-based primary care services, including preventive care, management of chronic diseases, wound care, management of substance use disorder, and management of mental health concerns.
• Responsible for providing treatment and care for patients seen by Street Medicine Kalamazoo in a trauma-informed and compassionate manner.
• Comfort with and/or willingness to learn about management and treatment of substance use disorder and mental health concerns.
• Able to perform and/or willing to learn minor outpatient clinical procedures, including phlebotomy, administration of intramuscular injections, incision and drainage, skin biopsy, etc.
• Orders and interprets diagnostic and therapeutic tests relative to patient’s specific needs.
• Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities.
• Monitors the effectiveness of all treatments and interventions.
• Responsible for the documentation of patient care per clinic standards.
• Acquires knowledge for new technology and policy/procedure revisions.
• Facilitates the patient’s transition within and between health care settings. Assists with hospital consults for unhoused patients and facilitates discharge planning.
• Collaborates with multidisciplinary team members by making appropriate referrals.
• Facilitates staff, patient and family decision making by providing educational tools.
• Maintains continuing educational unit credits as required for license.
• Availability to work occasionally on weekends and evenings to complete time sensitive projects.
• Attends and participates in weekly and monthly street medicine meetings.
• All other duties as assigned.

Qualifications

EDUCATION AND/OR EXPERIENCE: 

• Bachelor of Science, Nursing, from an accredited institution, required.
• Graduate from an accredited Nurse Practitioner program, required.
• Minimum 2 years clinical experience in primary care, addiction medicine, and/or psychiatry preferred.
• Experience working in home visiting programs preferred.
• Expereince with wound care preferred.

 

LICENSES, REGISTRATIONS, CERETIFICATIONS: 

• Licensed in the State of Michigan as a Nurse Practitioner or eligible to become licensed in the State of Michigan as a Nurse Practitioner required.
• Michigan Controlled Substance License is required.

OTHER SKILLS AND ABILITIES: 

• Knowledge and proficiency in working with the Electronic Health Record system (Epic and Cerner).
• Knowledge and proficiency in word processing and file management using Microsoft Office, Outlook and other software as required by the department.
• Knowledge and proficiency in grammar and spelling skills.
• Ability to effectively communicate with others verbally and in writing.
• Ability to interpret, comprehend, and transmit complicated and detailed instructions accurately.
• Demonstrates the ability to recognize priorities in organization of work flow.
• Able to perform duties independently, with a minimal need for direct supervision.
