Social Worker MSW

Under minimum supervision the CARE Network Social Worker provides highly skilled social work services on an intermittent basis to clients and their families. S/he assists patients and families with psychosocial and environmental problems that may predispose toward illness or are interfering with obtaining maximum benefits from medical care or reaching optimum level of physical functioning. The CARE Network provides community based comprehensive case management services for the chronically ill, which may include: CHF, Cancer, HIV/AIDS and other chronic diseases.

Required Qualifications:

• Master's Degree Social Work.
• 1 year Social work or counseling. 

Preferred Qualifications:

• upon hire: Clinical Social Worker License.

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
