About the role

Heritage Pointe of Warren is seeking an LPN/RN for a full-time days shift position working on our rehab unit. Must know all aspects of basic personal care, medication administration, treatment procedures, preventive, restorative, and therapeutic techniques. Must have knowledge of medial asepsis and sterile technique. Must have interest in and be able to communicate effectively with aged and ill residents with a variety of chronic illnesses or emotional disorders. Also, must be able to communicate effectively with visitors, supervisor, and co-workers, and work under the direct supervision of the charge nurse. Must be able to maintain confidentiality of residents’ conditions and personal affairs. Must be a graduate of a state approved school of nursing and hold a current state of Indiana license,

 

Responsibilities/Duties

  1. Assign all nursing assistants to residents, indicating break times and all special duties to be completed. Supervise work performance by checking CNA work closely to be sure all assignments have been completed.
  2. Administer all parenteral, intramuscular, and subcutaneous medications, including IV medications if properly trained in IV therapy.
  3. Round on all residents, paying attention to detail and utilizing individual health care plans.
  4. Ensure that individual resident care plans are followed.
  5. Assist in updating the care plan relating to the resident’s condition and level of care with the help of the IDT.
  6. Recommend changes needed in staffing according to the number of residents and their needs/acuity.
  7. Recommends conferences with the DON as needed and participate in staff meetings and in-service training to keep staff performance at optimum levels.
  8. Make rounds with the physicians and document visits in the EMR.
  9. Transcribe all physician orders ensuring that they are accurate and timely.
  10. Place orders for all medications and treatments.
  11. Ensure all medications and treatments are charted after given by the person administering the medication or treatment.
  12. If Qualified Medication Aids are on duty, be sure they are practicing within their limitations/scope of practice and facility policy.
  13. Direct charting on the shift so that all charting always reflects current progress and condition of residents.
  14. Responsible for the wellbeing and nursing care of all residents assigned to the unit during the tour of duty. Recognizes significant changes in condition of all residents and takes necessary actions to promote wellbeing and stabilize the condition of the residents.
  15. Supports resident families, guests, or legal representatives in a professional manner as concerns arise and as the situation requires.
  16. Recognize limitations when handling concerns/situations and ask for help when needed.
  17. Follow all facility policies and procedures and ensure that employees under your supervision are following them as well.
  18. Supervise the serving of prescribed diets, food & fluid intake, reporting persistent unresolved problems to the physician and Director of Nursing.
  19. Supervise the preparation of incident and accident reports as well as appropriate follow-up and notification.
  20. Ensure that copies of medical records are forwarded or sent with the transfer of a resident.
  21. Ensure that drugs covered by the Controlled Substances Act are verifiable by inventory.
  22. Detect and correct situations that have a high probability of causing accidents or injuries to residents.
  23. Reports noncompliance with policies, procedures, regulations, or breaches in confidentiality to appropriate personnel. Reports any retaliation or discrimination to HR or compliance officer.
  24. Reports any allegations of abuse, neglect, misappropriation of property, exploitation, or mistreatment of residents to supervisor and/or administrator. Protects residents from abuse and cooperates with all investigations.
  25. Reports any occupational exposures to blood, body fluids, infectious materials, and/or hazardous chemicals in accordance with facility policy.
  26. Participates in all life safety and emergency drills and trainings. Fulfills responsibilities as assigned during implementation or activation of the facility’s emergency plan.
  27. Reports work-related injuries and illnesses immediately to supervisor.
  28. Follows established infection control policies and procedures.
  29. As a condition of employment, completes all assigned training and skills competency.
