This is an exciting opportunity to join the Mass General Brigham Population Health department as a Project Specialist (Program Coordinator) within the GRACE (Geriatric Resources for Assessment and Care of Elders) program. The GRACE Program Coordinator will provide essential administrative and operational support to the GRACE interdisciplinary team, ensuring smooth coordination and communication across all aspects of the program.
Job Summary
As part of this initiative, the GRACE Program Coordinator will play a key role in:1) Providing professional office and administrative support to GRACE Interdisciplinary Team members.
2) Coordinating scheduling and logistics for in-home geriatric assessments conducted by nurse practitioners and social workers.
3) Supporting the development and documentation of individualized care plans using GRACE care protocols.
4) Facilitating communication between patients, caregivers, and care team members, including primary care providers.
5) Managing telephone support and assistance for older adults and their caregivers, ensuring timely and compassionate responses.
6) Organizing and supporting weekly interdisciplinary team meetings, including preparation of agendas and documentation of discussions and follow-ups.
This role is an essential part of Mass General Brigham’s commitment to improving care delivery for high-risk older adults through proactive, patient-centered management. The GRACE Program Coordinator will help ensure that care is well-coordinated, culturally responsive, and aligned with patient goals and preferences.
Key components of GRACE Team Care include:
- In-home assessments by a nurse practitioner and social worker team.
- Individualized care plans developed using GRACE protocols.
- Collaborative care planning with primary care physicians.
- Ongoing home-based care management and coordination.
- Weekly interdisciplinary team meetings including a geriatrician, pharmacist, and mental health liaison.
The GRACE Program Coordinator serves an integral role in the GRACE model and is considered a valued member of the GRACE Interdisciplinary Team.
Qualifications
What You'll Bring
Requirements:
- Bachelor's Degree in related field of study required.
- 1+ years of project management or project support experience required.
- Microsoft Office skills
Preferences:
- Experience working with Epic or other EHR strongly preferred.
- Basic financial/accounting skills preferred.
Knowledge, Skills and Abilities:
- Ability to prioritize, delegate, and manage multiple projects and responsibilities simultaneously.
- Strong oral and written communication skills.

- Occasional team meetings on Tuesdays at Assembly Row in Somerville, MA; likely no more often than once per month.
