Description

HSVS Special Medical Program is seeking a Senior Child Advocate - RN who will be responsible for a small caseload of child participants along with overseeing other Child Advocates in the program. Additional responsibilities will include clinical assessments during visits in the community, providing anticipatory guidance and health-related education to children and families; and assist with coordinating care for children living with medical complexity, or disabilities and their families. The Child Advocate provides these services during regularly scheduled visits to families' homes and other community sites, and on-site in our 29i Medical Practice. Care is delivered in collaboration with HSVS’s team of case planners, behavior specialist, mental health professionals, education specialists, physicians, nurse practitioners, and other staff. 

Primary Responsibilities and Essential Functions of Position:

  • Responsible for a small caseload of medically complex foster children.
  • Assessing medical records of each child by reviewing child’s diagnosis, birth history, inpatient/outpatient medical records, educational services, and mental health needs; includes pre-screening special medical youth in NYC Children’s Center
  • Delivery of sexual and reproductive health services to youth, and · Assessment for emotional and behavioral concerns and substance or alcohol use disorders; · Make referrals to community-based care and provide support as needed
  • Collaboration with Special Medical Case Planner, Supervisor, and Director. Train and provide medical guidance to foster and birth parents and support for their medical fragile children and family
  •  Assist in quality assurance activities and audits involving services provided to these children. Maintain up to date medical record documentation. · Oversee the training, performance and evaluation of Child Advocates in order to ensure maximum efficiency and effectiveness and adherence to requirements for record-keeping and documentation.
  •  Ensure policies, procedures, systems and functions required for the operation remain in compliance with all regulations issued by the Administration of Children’s Services (ACS), New York State Department of Health (NYSDOH), New York City Vaccines for Children (VFC) Program, Occupational Safety and Health Administrations (OSHA) and other regulatory organizations.
  •  Collaborate with staff at hospitals, outpatient practices and other child welfare agencies in the care of children and families enrolled in Special Medical Program.
  •  Accompany children and families to selected primary care and pediatric and surgical subspecialty visits, as needed; · Ensure accurate and timely documentation of all patient care and community-based activities.
  •  Provide telephone triage and advice to parents
  • Assist in quality improvement activities involving services provided to children in assigned caseload, including maintaining up-to-date medical record documentation.
  •  Perform any other duties as may reasonably be required to fulfill the responsibilities of the position, meet the needs of the agency and ensure compliance with regulations.

Qualifications

  • Registered Nurse (RN) license required.
  • Associate Degree in Nursing preferred.
  • Must have availability to work evenings and some weekends.
  • Demonstrate knowledge, understanding, and respect for diverse cultural norms and traditions with clients and co-workers at all times.
  • Be able to multi-task and have excellent organizational skills.
  • Have excellent interpersonal and communication skills.
  • Have excellent writing skills and competency managing databases
