SUMMARY

The Physical Therapist - Rehab assesses and restores patient's function, alleviates pain, and prevents disabilities by designing, planning, and implementing medically prescribed physical therapy interventions.

ATTRIBUTES

• Master’s Degree in Physical Therapy or Bachelor’s degree with prior experience as a licensed Physical Therapist required.

• Current New York State license or a limited permit as a Registered Physical Therapist

• Prior Physical Therapist experience preferred

RESPONSIBILITIES

• Treatment & Planning. Meet patient goals and needs by providing quality, evidence-based care; assess, develop and implement an individualized treatment plan; help the patient accomplish treatment plan and accept therapeutic devices by administering manual exercises; instruct, encourage and assist patients in performing physical activities

• Discharge Management. Assure continuation of therapeutic plan following discharge by designing home exercise programs; instruct patients, families and caregivers in home exercise programs; recommend and provide assistive equipment; recommend outpatient or home health follow-up programs

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant.
