Pharmacy Resident PGY2

• Functions as a pharmacist to perform basic responsibilities in compliance with Board of Pharmacy, Drug Enforcement Agency (DEA), and The Joint Commission (TJC)
• Prepares, dispenses, and manages medications to support safe and effective drug therapy for patients.  Collaborates with the health care team to provide comprehensive medication management to patients with infectious diseases following a consistent care process
• Provides general and patient specific medication information to patients, providers, nurses and other health care professionals. Interacts routinely with physicians and nurses to provide optimal pharmaceutical care to patients.
• Counsels patients in medication attributes and the proper use of medications
• Participates in patient care rounds with multi-disciplinary teams to make pharmaceutical interventions and evaluate patient progress.
• Conducts and evaluates medication histories, assesses compliance and suggests modifications so as to achieve desired outcomes.

• Collaborates with the health care team to provide safe and effective patient care to a diverse range of patients, including those with multiple co-morbidities, high-risk medication regimens, and multiple medications following a consistent patient care process.

• Ensure continuity of care during patient transitions between care settings.
• Manage antimicrobial stewardship activities and participate in the hospital’s antimicrobial stewardship program and committee

ADVANCING PRACTICE and IMPROVING PATIENT CARE

• Participates in collection of data for medication utilization evaluations (MUEs)
• Demonstrates the ability to manage formulary and medication-use processes, as applicable to the organization and antimicrobial stewardship program
• Serve on hospital committees and present material to committee members as needed (e.g. Pharmacy & Therapeutics Committee, Antibiotic Stewardship Committee, and other related committees)

• Complete a research project by demonstrating the ability to evaluate and investigate practice, review data, assimilate scientific evidence to improve patient care and/or the medication use system.  Research project should seek to advance the pharmacy profession or infectious diseases pharmacy and be suitable for publication

TEACHING, EDUCATION, and DISSEMINATION OF KNOWLEDGE

• Employs appropriate preceptor roles when engaged in teaching (e.g. PGY1 pharmacy residents, pharmacy students, technicians, or other health care professionals). 
• Provides educational in-services for pharmacy and multidisciplinary staff as necessary
• Provides effective medication and practice-related education to infectious diseases patients, caregivers, health care professionals, students, and the public (individuals and groups)
• Accurately defines educational needs with regard to target audience (e.g., individual versus group, patients with infectious diseases, etc.) and learning level (e.g., health care professional versus patient).

• In addition to clinical services, the residents are expected to employ appropriate preceptors’ roles when engaged in teaching (e.g., PGY1 residents, students, pharmacy, technicians, or other health care professionals). They serve as preceptors throughout the year to pharmacy students completing their clinical clerkships and as educators for in-services to the staff (e.g. pharmacist, technicians, nurses, physicians).

LEADERSHIP and MANAGEMENT

• Staffing is a requirement for all residents and based on specified weekend schedule provided at the start of the residency program.
