Description:

Join our mission-driven team at MercyOne VNA and make a lasting impact—one home at a time. We're seeking a compassionate, licensed Physical Therapist to deliver high-quality, in-home therapy to patients of all ages. You’ll support recovery and mobility while working closely with families and care teams across our community.

What You'll Do:

• Provide in-home physical therapy for pediatric to geriatric patients

• Develop individualized care plans and complete timely documentation

• Collaborate with interdisciplinary teams and supervise support staff as needed

• Travel within assigned service area (mileage reimbursement provided)

• 20 hours per week (.5 FTE)

What You'll Need:

• Bachelor’s degree in Physical Therapy (or higher)

• Licensure in Iowa or Illinois at hire; dual-state licensure within 6 months

• Valid driver’s license and auto insurance (Genesis minimums)

• CPR certification (BLS)

• Home care experience preferred—but not required
