Bright Start, a program of Children's Home Society in Sioux Falls, SD, is seeking a Full Time Bright Start Nurse Home Visitor to help provide comprehensive community health services for high-risk women and children.


What You'll Do:

Your responsibilities as a Full Time Bright Start Nurse Home Visitor:

  • Client Relations: Plan and facilitate home visits with program participants in accordance with Bright Start and Nurse Family Partnership principles. Assess participant needs as defined in the protocol, setting measurable and achievable goals and plans.
  • Public Relations: Provide appropriate referrals to community resources. Assist with marketing and representing the program in the community.
  • Compliance: Obtain and record client data according to Nurse Family Partnership model and Agency protocol. Complete charting and documentation of client visits and interactions within 2 business days. 
  • Professional Development: Complete required training. Participate in Continuous Quality Improvement projects.

Schedule:

*This is a remote work through home visitations while officing from home. Company vehicle provided.

Monday-Friday daytime hours, but flexible schedule is required. 40 hrs/week.

This position primarily serves the counties of McCook, Lincoln, Turner, and Minnehaha


Compensation: Up to $40.00/hr base pay (Depending on Education/Experience)+ up to $2/hr shift differential.

Requirements

Experience/Education/Skills:

  • Bachelor’s degree in nursing preferred.
  • Valid nursing license in the State of South Dakota.
  • Previous experience in women’s health care, pediatrics, home visitation services, assessments and case management preferred.
  • Must possess valid driver’s license.


Physical Demands and Work Environment:

Regularly in normal office environment. Area travel and client home visits regularly
