Home Health Registered Nurse (RN)

Touchstone Health of San Antonio is actively seeking a qualified and compassionate Registered Nurse (RN) to care for our patients. The ideal candidate has excellent clinical skills, a passion for direct patient care, and the ability to function well autonomously. Responsibilities & Duties:

• Evaluate and treat patients using the most current technology and practices
• Communicate with the physician to decide the initial plan of care based on initial patient assessment & physician’s orders
• Successful management of a consistent case load
• Participate in clinical outcomes monitoring, follow up, and agency performance improvement initiatives
• Manage and educate Home Health Aides and LPNs performance in implementing nursing services
• Ability to work in a collaborative environment with an interdisciplinary team
• Travel the San Antonio area and surrounding area

Requirements

Skills & Requirements:

• Current and unencumbered license to practice as a Registered Nurse in the state of Texas
• Minimum of one (1) year of nursing experience
• Current CPR certification
• Reliable transportation, valid driver's license with no DUI's, revocations, or suspensions in the last 5 years
• Exceptional customer care
• Excellent written and verbal communication skills
• MS Office skills (Word, Excel, PowerPoint)
