DaVita is seeking a full-time Nurse Practitioner (NP) Manager to join our IKC Care Team supporting PNW. This role provides clinical leadership and management for Nurse Practitioners within a designated region, ensuring the delivery of high-quality, cost-effective care for patients with Chronic Kidney Disease (CKD) and End-Stage Kidney Disease (ESKD).
The NP Manager functions autonomously while partnering closely with clinical and operational leadership to support population health management, quality patient outcomes, and the overall sustainability of the program.

Key Responsibilities
Clinical Leadership & Team Management
Serve as the clinical leader and direct manager for a team of Nurse Practitioners based in Oregon and Washington, providing oversight, coaching, supervision, and performance management.
Serve as the clinical leader of a care pod (including an RN and Care coordinator) to provide direct support to a panel of Oregon CKD patients.
Clinical Care & Evaluation: The primary responsibility is completing Comprehensive Health Evaluations (CHEs) to assess and manage comorbid conditions while addressing medical, social, emotional, and financial needs. Reduce hospitalizations and improve patient outcomes.
Ensure high standards of clinical quality and productivity, including accurate documentation, appropriate diagnoses, closure of care gaps, reduction of preventable hospitalizations, and compliant coding practices.
Support recruitment efforts and lead onboarding and training for newly hired Nurse Practitioners.
Provide ongoing clinical education and guidance to enhance NP expertise in CKD, ESKD, and commonly associated chronic conditions.
Strategy, Quality & Population Health
Partner with Medical Officers and Operations Leadership to develop and execute clinical and operational strategies.
Identify opportunities to streamline workflows and prioritize care for high-risk, high-utilization patient populations.
Lead complex case reviews and manage escalations involving high-risk or high-utilization patients, collaborating with interdisciplinary teams and external resources as needed.
