We seek individuals who are driven to make a difference and embody our motto, “To Care is an Honor.” Join Ennoble Care today!</p></div><p><strong>Ennoble Care is seeking a full-time <em>Patient Care Coordinator</em> to join our team in an office-based setting in <em>Voorhees, New Jersey</em>. This role is an integral, in-office member of our clinical care team, working alongside fellow Care Coordinators and collaborating with Ennoble Care providers within a family-practice-style model of care.</strong></p> <p><strong>While not patient-facing, the position plays a critical role in supporting patient-centered care coordination, documentation, and communication, helping ensure continuity of care and timely follow-up across care settings.</strong></p> <p><strong>At Ennoble Care, we foster a positive, respectful culture rooted in teamwork, where individuals are valued and encouraged to do meaningful work while maintaining a healthy work-life balance. We support internal advancement and long-term career growth based on performance, organizational needs, and role availability.</strong></p> <hr> <p><strong>KEY RESPONSIBILITIES</strong></p> <ul> <li>Coordinate patient-centered care activities using Ennoble Care’s electronic medical record (EMR) system</li> <li>Support Ennoble Care providers and clinical teams through accurate documentation, follow-up tracking, and care coordination</li> <li>Serve as a key point of communication between providers and internal care teams regarding clinical updates, patient needs, and care coordination priorities</li> <li>Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely and appropriate manner</li> <li>Assist with care transitions, referrals, and coordination with external partners, as needed</li> <li>Collaborate closely with fellow Patient Care Coordinators and internal care team members within an office-based environment</li> <li>Maintain accurate, timely documentation in accordance with established workflows, documentation standards, and escalation protocols to support time
