Physical Therapist

The Physical Therapist is directly responsible for planning and administering medically prescribed physical therapy treatment programs in the home settings for patients to restore function, relieve pain and prevent disability following disease, injury or loss of body parts. The physical therapist consults with the physician in developing a plan of treatment including specific procedures and equipment to be used and frequency of visits.

Required Qualifications:
- Degree in Physical Therapy
- Washington Physical Therapist License upon hire
- National Provider BLS - American Heart Association within 30 days of hire
- National Provider Identifier (NPI) number within 30 days of hire
- 1 year Home Health

Preferred Qualifications:
- 2 years of Physical Therapy with adults with physical disabilities
- 1 year of experience with the geriatric population
The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
