An exciting opportunity presents itself for YOU to join the talented team of health professionals and support staff within the newly created 24/7 Hospital in the Home North service.

Hospital in The Home’s sole purpose is to provide exceptional hospital level health care and ongoing support to the residents of Northern Tasmania to negate the need for hospitalisation in our mainstream facility and enable patients to remain in their own home and community under the watchful/vigilant eye of a dedicated health team. 

Our exciting new service has recently launched, and we are able to recruit to multiple permanent, full time/part/time and relief positions within this critical service.  We currently have a diverse mixture of key positions available, ranging from Medical Staff Specialists, Senior Medical Registrars, Nurse Unit Managers, Associate Nurse Unit Managers, Nurse Practitioners, Registered Nurses, Allied Health professionals (incorporating Senior Social Workers, Senior Physiotherapists and Senior Occupational Therapists) to a dedicated full time Program Support Officer.

The Role:

As part of Hospital in the Home (HITH) – North provide hospital level care within the patients’ home or community, allowing for earlier transition home from the hospital environment and / or complete hospital substitution for suitable patients. 

Provide specialist clinical assessment and management of patients in the community setting including rapid interventions for acute health issues and chronic exacerbations, preventing unnecessary hospital admissions. 

Supports the efficient and effective provision of care, based on clinical standards and best practice principles within a collaborative and multidisciplinary framework to support individuals with acute care needs in the community.

Promote and participate in the development and delivery of best practice hospital in the home medical services in Tasmania, including involvement in undergraduate and postgraduate teaching and active involvement in research.

Duties:

1. Clinical Role:

• Be responsible for direct specialist medical treatment and care to patients admitted to HITH and available for consultation and patient review when clinically appropriate.   
• Provide expert clinical consultancy, review, advice and support to other members of the multidisciplinary team. 
• Participate in an after-hours consultant on call roster for HITH service.

1. Service Role:

• Participate in conjunction with other members of the multidisciplinary team in continually designing, evaluating and improving the standard and outcomes of hospital in the home services provided.
• Provide active leadership, mentoring and supervision to medical colleagues.
• Ensure complaints are recorded and investigated in accordance with Departmental guidelines.
• Be involved in peer review and quality improvement activities. 
• Ensure ethical and medico-legal considerations and reporting are observed in all aspects of patient management.

1. Teaching:

• Participate in teaching programs of medical, nursing and health professional undergraduate and postgraduate student.
• Maintain an active role in supervision and coordination of a program of clinical research activities appropriate to hospital in the home throughout the North.
