Job Summary: Patient Placement Coordinator is an experienced Registered Nurse who is critical to the facilitation and decision making of inbound patient transfers from outside hospitals to our health system. The focus of the position will be with regards to facilitation of the patients going to services in the Department of Medicine (Cardiology, Hematology, Oncology, Nephrology, Geriatrics, General Medicine and Hospital Medicine). This position requires critical thinking skills, an operational knowledge of the services provided by the Medical Center, and the ability to act as a liaison between the Medical Center and referring hospitals. Responsibilities include facilitating appropriate placement of patients by planning, coordinating, and expediting patient throughput. This position utilizes a multidisciplinary team approach to successfully collaborate with medical staff, hospital employees and referring physicians. The schedule will be days to start but since the Patient Operations Center is open 24 hours per day, 365 days per year, this position may be required to work a flexible schedule including days, evenings, nights, weekends, and holidays, as well as during inclement weather in the future. This position is severe weather critical.
Education/Experience: Graduate of an accredited school of nursing for registered nurses, BSN preferred; 3-5 years medical -surgical hospital experience; varied experience preferred, especially emergency room and/ or critical care.
Reports To: Manager of Patient Flow Operations Center. Licensure, Certification and/or Registration: Current licensure to practices as a Registered Nurse in the state of North Carolina; BCLS certification preferred; valid drivers license
Essential Functions: 1. Adheres to general hospital standards to promote a cooperative work environment by utilizing communication skills, interpersonal relationships and team building. 2. Serves as the liaison between the medical staff of the referring facility and the medical staff of WFBH to ensure a smooth inbound patient transfer process. 3. Provides guidance to referring facilities as to the most appropriate service or medical specialty to address their patients medical needs. 4. Interacts with Patient Logistics staff to ensure appropriate and timely placement of patients accepted for transfer to the Medical Center. 5. Interacts with coworkers in an effective, positive and professional manner to encourage a cohesive work environment. 6. Engages in efficient assessments with outside providers to identify the appropriate service for the patient to be assigned to, based on bed and service availability. 7. Develops and maintains effective processes and documentation in information systems to facilitate efficient and appropriate admissions. 8. Demonstrates responsibility for professional and personal development. 9. Interacts frequently with the Care Coordinator in the Operations Center to identify potential patients who can be transferred from WFBH to one of our system hospitals.
Skills and Qualifications: Previous experience in a call center preferred. Excellent customer service skills. Experience in multidisciplinary team collaboration preferred ? Excellent telephone speaking voice and listening skills. Excellent knowledge of the nursing process and critical thinking skills. Basic computer skills with ability to navigate within multiple screens comfortably. Typing speed of 35 wpm minimum. Excellent organizational skills and the ability to work within a fast paced environment
